Our team communicates with the patientís physician prior to and after discharge, maximizing continuity of care and helping to facilitate a smooth transition as the patient returns to the nursing home.

Direct-admit patients can be cared for by our multidisciplinary geriatric team, which maintains a continued flow of communication with the nursing home medical director. Alternatively, nursing home medical directors can provide care to the patient with support from our team.
